While there is no form or application required for your child to receive this benefit, it is vital for state reporting mandates that the district maintain current records. Therefore, we kindly request that you fill out the Parent Form and return to the school at your earliest convenience. Students received this form with their beginning of the year packet but should you need an additional copy you can print it and mail it to the school, send it in with your child, or email it to ssmith@slwildcats.org.
